The Jackson Care Hub empowers healthcare providers and community organizations to collaborate on client care. By connecting clinical and social service partners through a shared, secure IT platform, the Hub allows clients to be directly connected to the resources they need.
The Hub provides critical infrastructure for coordinated care, helping providers make timely referrals, track follow-through, and identify service gaps. This integrated approach strengthens care across the community, improves access to needed resources, and drives better outcomes for the people of Jackson.
Meeting Today’s Need
The Jackson Care Hub is an integrated referral platform. Our population-based approach allows clients to be screened across hundreds of service locations. Our Community Health Workers provide compassionate, human-centered care to everyone. Our resource directory ensures our clients have access to accurate information about existing services. And our network of partner agencies use the Hub to receive referrals and connect directly with clients to address their needs.
Our community agencies offer many wonderful services. But people often struggle to find the right resources. This is especially true in times of crisis when clients feel overwhelmed and scared. The Jackson Care Hub connects clients directly to the right resources at the right time. This increases the likelihood that our agency partners will be able to provide the support clients need when they need it.

Meeting today's needs is only part of the solution. We do have resources but there are still people we can't help. There are still needs that cannot be met. Jackson, like many communities, does not have enough affordable housing options. Jackson, like many communities, does not have a systemic solution to food insecurity. These issues are not attributed to any one agency. It is the collective responsibility of the community to address these issues so that everyone in Jackson can thrive.
The Jackson Care Hub generates powerful data about when and where existing systems fail. By tracking social needs in real time, and monitoring the resources available to meet those needs, the Hub can identify emerging trends and service gaps across the community. By surfacing these patterns, we help partners see beyond the individual cases to the system-wide dynamics that affect our people. These insights equip decision-makers with the information necessary to design better systems, shift resources, and leverage investments to maximize impact. In Jackson, we do more than respond.
