Hi! I’m Megan Albertson, Director of the Jackson Care Hub. I’ve spent the past 14 years working with wonderful folks in our community, all of whom are dedicated to improving the health and wellbeing of our people. I am from Jackson. I am proud of Jackson. And today, more than ever, I am hopeful for Jackson.
From that place of hope, I am excited to introduce the new Jackson Care Hub brand and website—a space created to tell our story and celebrate the incredible work happening in our community!
At the Jackson Care Hub, we believe health begins in the community.
Our goal is to connect healthcare providers and social service agencies, empowering them to work together to ensure everyone in Jackson has access to the support and resources they need to thrive. The launch of this new website is a celebration of what we have accomplished so far.
We collaboratively designed a shared IT platform, which is no small feat for anyone used to participating in community meetings!
We developed an amazing team of community health workers who support everyone in Jackson seeking help.
We established interoperability protocols to securely share narrowly defined data assets across health and social services providers to improve coordination of care for shared clients.
We created methods to analyze and distribute important information about emerging needs and developing resources to support community leaders in creating systems of care that address the root causes of material scarcity and poor health.
I am very proud of what we have achieved together. And yet, we have more work to do and more stories to tell. We are committed to serving Jackson and sharing our efforts freely in active solidarity with anyone striving to create a world where every single person can thrive.
